High School Players School/Residence Verification
Acceptable school verification can be a school class schedule or a screenshot of the current school ID with the player’s name, the school, the academic term, visible and legible. Team eligibility is based upon the school where the student is enrolled as a full-time student. Full-time student is determined by UHSH as it varies across school districts. For online, home-schooled and charter school students, they will upload documentation of the program in which they are enrolled and a document that verifies the home residence of the player. A parent’s utility bill with the home residence/address visible and legible for example.
If a birth certificate is needed, you will be contacted with instructions for submission.
Middle School Players School/Residence Verification.
Acceptable Middle School player verification can be proof of school registration, report card, a school class schedule or a screenshot of the current school ID with the player’s name, the school, the academic term, visible and legible. The student name, school name, academic term must be visible and legible. Team eligibility is based upon the school where the student is enrolled as a full-time student. Full-time student determined by UHSH as it varies across school districts. For online, home-schooled and charter school students, they will upload documentation of the program in which they are enrolled and a document that verifies the home residence of the player. A single document with school/institution/program enrollment and residential address of the player will suffice. A parent’s utility bill with the home residence/address visible and legible is acceptable for the residence proof.

Submission of false documentation or false information may result in ineligibility of the player and sanctions for the Trustee, Coach, Parent, and Team.
